Life-Long Behavior Therapy LLC - Terms of Service

Welcome to Life-Long Behavior Therapy LLC ("LLBT," "we," "us," or "our"). These Terms of Service ("Terms") outline the terms and conditions governing your use of our Applied Behavior Analysis (ABA) therapy services and any related products, materials, or content (collectively, the "Services"). By using our Services, you agree to comply with and be bound by these Terms. If you do not agree with these Terms, please do not use our Services.
2. Services
LLBT provides ABA therapy services for individuals with behavioral challenges, developmental disabilities, and related needs. The Services may include assessment, intervention planning, therapy sessions, progress monitoring, and related support.
3. Client Responsibilities
By using our Services, you agree to:
Provide accurate and complete information about the individual receiving therapy.
Abide by the therapy plans and recommendations provided by our qualified professionals.
Attend scheduled therapy sessions and provide necessary cooperation.
Notify LLBT of any changes in medical history, behavioral concerns, or relevant information.
Respect the privacy and confidentiality of all parties involved, including other clients and staff.
4. Privacy and Confidentiality
LLBT values your privacy and takes the confidentiality of your personal and clinical information seriously. We maintain industry-standard practices to protect your data. However, please understand that no method of transmission over the internet or electronic storage is entirely secure.
5. Payment and Billing
Payment for Services will be discussed during the initial consultation. LLBT may bill you directly or work with your insurance provider, as applicable. Failure to make timely payments may result in a disruption of services.
6. Cancellation and Rescheduling
If you need to cancel or reschedule a therapy session, please provide us with sufficient notice. Late cancellations or no-shows may impact the continuity of the therapy plan.
7. Termination of Services
LLBT reserves the right to terminate or suspend Services if there is a violation of these Terms, non-compliance with therapy plans, or other disruptive behavior that affects the well-being of staff or other clients.
8. Intellectual Property
All materials, content, and resources provided as part of the Services are the intellectual property of LLBT and are protected by applicable copyright, trademark, and other intellectual property laws.
9. Limitation of Liability
LLBT shall not be liable for any direct, indirect, incidental, consequential, or special damages arising out of or in connection with the use of our Services, regardless of the cause of action.
10. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the state of Indiana, without regard to its conflict of law principles.
11. Changes to Terms
LLBT reserves the right to modify or update these Terms at any time. Continued use of our Services after any such changes constitutes your acceptance of the modified Terms.
